MBS Basketball has my permission to use my photograph, video and audio
recordings in future publications, web pages, and other promotional materials produced. I understand the circulation of the materials could be worldwide and that there will be no compensation to me for this use.
Medical Clearance
I authorize the employees of MBS basketball camp to manage and provide first aid to my child in case of injuries. I authorize the transportation by ambulance or other means to a hospital center and I agree to pay for the expenses.
Refund Policy
No refund (Only a medical reason will be considered).
